The process is easy!

1. Call 208.514.2500 and press 1 for an appointment. Our staff will be happy to schedule at your convenience with one of our physicians or behavioral health staff.
2. If you are a new patient to us, simply fill out and submit the form linked below from your device. Our staff will contact you with details on how to sign into your appointment from home.

Pharmacy:

Additionally, if you use our pharmacy to pick-up your prescriptions, our staff will deliver your prescriptions to your vehicle in the parking lot. We also have options to mail your medications, please call our pharmacy for more details.

If you would like to utilize one of our internal pharmacies to conveniently pick up any prescriptions after your appointment, simply have your physician send your prescriptions to:
